Terms and Conditions for the Purchase and Redemption of Gift Vouchers at SALON Day Spa

Definitions:

1. 1. Buyer – a person purchasing a Gift Voucher,
   2. Recipient – the holder of the Gift Voucher indicated by the Buyer during the process of placing an order for the Gift Voucher; a person with full legal capacity,
   3. Service Provider – GT52 Sp. z o.o. with its registered office in Poznań (60-311) at ul. Grunwaldzka 52, NIP: 779-255-98-89,
   4. Gift Voucher – a term bearer document for a specific treatment (Voucher) and/or a package of treatments (Gift Card) for a specified amount purchased by the Buyer for the Recipient,
   5. Gift Voucher Terms and Conditions – these Terms and Conditions for the purchase and redemption of Gift Vouchers,
   6. Terms and Conditions of Use of Services – Terms and Conditions of Use of Services at Good Time Institute&Medical Spa applicable at the Service Provider

   7. Salon – the place where the Gift Voucher is redeemed at the Service Provider’s

2. GENERAL
   1. The Gift Voucher has its own individual identification number.

   2. Gift Vouchers can be purchased in the SPA Salon.
   3. The following forms of payment for the Gift Voucher are accepted at the SPA:
      1. card payment,
      2. payment in cash at the SPA ticket office
      3. payment by bank transfer to the SPA account.
   4. If the option is selected – payment by bank transfer, the Buyer is obliged to pay before collecting the Gift Voucher. Payment by bank transfer is deemed to have been made on the day the required amount is credited to the SPA account.
   5. The Gift Voucher will be issued to the Buyer in the SPA Salon immediately after the Buyer has paid – by payment card, cash or bank transfer – the amount equivalent to the declared amount of the Gift Voucher.
   6. After making the payment, the Buyer receives one of the documents confirming its execution, i.e.:
      1. receipt
      2. Invoice.
   7. If the Buyer wishes to receive an invoice or receipt with a tax identification number, the Buyer is obliged to send the recipient’s data in advance, i.e. Tax identification number, or company designation, address (for invoices).
   8. Gift vouchers entitle you to use the services in accordance with the price list applicable at the Service Provider on the day of the service and include prices without promotions and discounts.

3. TERMS AND CONDITIONS FOR REDEEMING A GIFT VOUCHER
   1. The Gift Voucher entitles the Recipient to make a payment for a specific treatment (Voucher) or for any treatment of their choice (gift card) or preparation from the Service Provider’s offer at the regular price, not including discounts (unless otherwise stated).
   2. The Recipient is obliged to book a convenient date of the visit in advance (at least one week before the planned visit) by providing the Voucher/Gift Card number (located on the Gift Voucher) at the Salon’s telephone number or in person at the Salon.
   3. The gift voucher must be presented before the treatment in the Salon.
   4. The expiry date can be found on the Gift Voucher.
   5. The gift voucher is valid for 3 or 6 months from the date of purchase. The Gift Voucher must be redeemed during this period. After the expiry of the validity period of the Voucher, in whole or in part, the unused value is lost.
   6. A gift voucher can only be used by the Recipient.
   7. If the reservation is not cancelled by phone or in person, 24 hours before the scheduled visit, the gift voucher will be deemed to have been redeemed.
   8. In the event that the value of the service and/or the purchased preparations is lower than the value of the Gift Voucher, the Service Provider is not obliged to refund the difference in value.
   9. In the event that the value of the service and/or the purchased preparations exceeds the value of the Gift Voucher, the Recipient pays the difference.
   10. If the Gift Voucher includes treatments that cannot be performed at the Recipient’s due to contraindications, it is possible to exchange them for a treatment of a similar, but not higher value according to the list price, not taking into account other discounts.
   11. Gift vouchers cannot be combined with promotions and discounts available from the Service Provider.
   12. After prior consultation, the Gift Voucher can be redeemed during several visits to the Service Provider.
   13. A gift voucher can only be redeemed with the Service Provider.
   14. A gift voucher can only be used to pay for services during its validity period.
   15. A gift voucher purchased in a promotion cannot be used to pay for goods or cosmetics available from the Service Provider in the Salon.
   16. A gift voucher purchased in a promotion cannot be used for aesthetic medicine treatments or permanent make-up services.
   17. A gift voucher purchased in a promotion cannot be used for injectable treatments (Mesotherm, Vital Injector, Tissue Stimulators, Demapen).
   18. The packaging of the Gift Voucher and the extras (freebies) contained therein depend on the value of the Gift Voucher
   19. The Gift Voucher Terms and Conditions are an integral part of the Gift Voucher sales agreement.
   20. The condition for the provision of the service by the Service Provider is the acceptance of these Terms and Conditions by the Buyer.

4. RETURNS
   1. Vouchers are non-refundable and cannot be exchanged for cash.
   2. Any complaints related to the purchase of treatments using the Voucher will be considered by the Service Provider in accordance with the procedure for returning the treatment or treatment package described in the Terms and Conditions of Use of Services at Good Time Medical Spa.

5. PROTECTION OF PERSONAL DATA
   1. As part of the performance of the contract by the Service Provider, personal data of natural persons are processed within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016. on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and the adoption of Directive 95/46/EC (General Data Protection Regulation) “GDPR”.
   2. The administrator of personal data is – GT52 Sp. z o.o. with its registered office in Poznań (60-311) at ul. Grunwaldzka 52, NIP: 779-255-98-89.
   3. The Service Provider takes all necessary actions required by the provisions of the GDPR as well as by national regulations in order to ensure an appropriate level of protection of the processed personal data. In particular, it shall apply the security measures referred to in 32 et seq. GDPR.
   4. Detailed information on the principles of personal data processing is included in the Terms of Use and Privacy Policy.

   4. The Buyer and the Donee may communicate with the Service Provider using the above-mentioned contact details.
   5. The Buyer and the Donee may contact the Service Provider by phone from Monday to Friday from 9 a.m. to 9 p.m., and on Saturday from 9 a.m. to 5 p.m.

7. FINAL PROVISIONS
   1. The contract for the sale of Gift Vouchers is concluded in Polish.
   2. These Terms and Conditions are available in the Service Provider’s Showroom and on the www.goodtimemedicalspa.pl’s website.
   3. The Service Provider reserves the right to amend the Gift Voucher Terms and Conditions for important reasons, in particular in the event of a change in the law, a change in payment and delivery methods, if these changes affect the implementation of the provisions of the Terms and Conditions. The Service Provider will inform about any changes to the Terms and Conditions on the website at www.goodtimemedicalspa.pl and in the SPA Salon.
   4. To the extent not regulated by the Gift Voucher Regulations, the Regulations for the use of services at Good Time Medical Spa of 1.04.2019. and the Privacy Policy of 1.04.2019. shall apply mutatis mutandis. The above-mentioned documents can be found on the www.goodtimemedicalspa.pl website
   5. In matters not covered by the Terms and Conditions, the generally applicable provisions of Polish law shall apply.
